Our client is actively seeking an experienced Family or Adult Nurse Practitioner to support their busy post acute care facility. This is a flexible part time opportunity. 1 to 2 days per week you choose the schedule.

Key Responsibilities:

• Patient Care : Provide direct clinical care to residents including assessment diagnosis treatment and ongoing management of medical conditions.
  
• Comprehensive Assessments : Conduct physical exams review medical histories order and interpret diagnostic tests and develop treatment plans tailored to individual patient needs.
  
• Medication Management : Prescribe and manage medications including pain management antibiotics and other therapies as appropriate for postacute conditions.
  
• Care Coordination : Collaborate with attending physicians nurses physical therapists social workers and other healthcare providers to coordinate care plans and ensure effective transitions of care.
  
• Family Education and Support : Educate patients and families about treatment plans medications and lifestyle modifications to promote recovery and wellness.
  
• Clinical Documentation : Maintain accurate uptodate electronic medical records in compliance with facility and regulatory standards.
  
• Chronic Disease Management : Monitor and manage chronic conditions such as diabetes hypertension COPD heart disease and others commonly encountered in postacute care settings.
  
• Quality Improvement : Participate in quality improvement initiatives to enhance patient outcomes and ensure compliance with healthcare standards and best practices.
  
• OnCall Responsibilities : Includes some afterhours or weekend coverage to address urgent medical issues.
  

Requirements

• Education : Master s degree in Nursing (Family Nurse Practitioner or Adult Nurse Practitioner program).
  
• Licensure : Current unrestricted license to practice as an FNP or ANP in the state of Arizona.
  
• Certifications : Board Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP).
  
• Experience : 0 5 years of experience providing direct patient care; new graduates encouraged to apply
  
• Skills :
  • Strong clinical judgment and decisionmaking abilities.
    
  • Excellent communication and interpersonal skills.
    
  • Ability to work effectively as part of an interdisciplinary team.
    
  • Proficiency in using electronic health records (EHR) and medical documentation systems.
